Here's an instructive six and a half minute video for anyone who's struggled with this issue. It's produced by photographer Dom Bower. I originally saw it on FStoppers.com, which provides an informative introduction to the video.
You'll notice that Dom uses only one flash, mounted on the shoe of the camera, and points it directly at the ceiling. This isn't about an elaborate multi-flash set up.
